As a Hospice Intake Coordinator for Interim HealthCare, you’ll serve as the central point of coordination for hospice referrals, ensuring smooth transitions and accurate documentation to support our caring clinicians.

Interim HealthCare, the nation’s first home care company, is hiring a Hospice Intake Coordinator to support intake operations across regional offices. If you’re passionate about connecting patients to the right care and thrive in a fast-paced healthcare environment, this role is for you.

Our Hospice Intake Coordinators enjoy:

  • $80,000 - 90,000/year
  • Schedule: Monday - Friday (8:00am - 5:00pm)

As a Hospice Intake Coordinator, you will:

  • Coordinate incoming referrals for Palliative and hospice services, ensuring timely follow-up, accurate data entry, and compliance with standards.
  • Serve as a key liaison between referral sources (physicians, hospitals, discharge planners), patients, families, and internal clinical teams.
  • Support intake workflows and processes to promote efficiency and consistency across regional offices.
  • Communicate with clinicians to confirm patient eligibility, insurance verification, and documentation requirements.
  • Maintain referral tracking systems and dashboards; assist agencies with daily report-outs as requested.
  • Collaborate with intake managers, nurses, and administrative staff to resolve referral issues and ensure smooth transitions of care.
  • Ensure proper use of systems such as Axxess EMR, Forcura, and online portals.
  • Participate in daily huddles with home health and hospice teams and provide updates on referral statuses.
  • Maintain knowledge of regulatory requirements, Medicare/Medicaid, and best practices in home health and hospice intake.

Qualifications:

  • Current, active Registered Nurse (RN) license in good standing (multi-state/compact preferred).
  • Bachelor’s degree in nursing or related field preferred.
  • Two or more years of experience in home health or hospice intake, admissions coordination, or clinical operations.
  • Strong understanding of home health and hospice services and eligibility requirements.
  • Highly organized with strong attention to detail and time management skills.
  • Excellent communication and customer service abilities.
  • Proficiency in electronic health records and standard office software.

Working Conditions & Physical Requirements:

  • Primarily office-based with regular computer and phone use.
  • Flexibility to assist with coordination needs outside normal business hours.
  • Light physical activity; occasional lifting up to 10 lbs.
