Hospice Community Liaison

We are seeking a high-performing Hospice Community Liaison with a proven ability to grow census and build strong referral relationships.

This role is ideal for an experienced hospice or healthcare marketer who knows the territory, understands the referral landscape, and can hit the ground running.

Territory:

• Gwinnett, DeKalb, Forsyth, and surrounding areas

What You’ll Do:

• Drive census growth by building and maintaining strong referral relationships
• Develop and expand partnerships with physicians, hospitals, SNFs, ALFs, and case managers
• Execute a strategic call plan to generate consistent referrals
• Conduct patient/family meetings to educate on hospice services and secure admissions
• Represent the organization in the community through networking and events
• Identify new business opportunities and grow market presence

What We’re Looking For:

• Proven track record in hospice or healthcare sales/marketing
• Existing relationships in the local healthcare community strongly preferred
• Demonstrated ability to meet or exceed admission and census growth goals
• Highly self-motivated, disciplined, and results-driven
• Excellent communication and relationship-building skills
• Strong organizational and time management abilities

Compensation:

• Competitive base salary (based on experience)
• Uncapped monthly & quarterly bonus potential tied to performance
