We are seeking a compassionate and experienced Medical Social Worker to join our team on a per diem basis , providing coverage as needed and supporting patients and families through critical moments of care and transition.

Position Purpose: Provides evidenced based interventions for patients within the program. In collaboration with the multidisciplinary healthcare team, the Counselor works collaboratively with the patient’s care team to allow for timely and quality care for mental health and substance use disorders. Position is FLSA exempt.

Hours/Shift: Part-Time (PRN) / Minimum of 1 shift a week.

Position Purpose: Under the direction of the designated physician and clinical care providers, work collaboratively with multidisciplinary team members, patients, families, and community agencies to identify appropriate community resources to achieve desired patient outcomes.

Key Responsibilities:

Care Coordination & Case Management

  • Collaborate with physicians, nurses, case managers, and care teams to support high-risk patients
  • Provide education to patients and families on insurance and regulatory processes (AHCCCS, Medicare)
  • Serve as a liaison between patients, families, and healthcare teams
  • Maintain accurate and timely documentation

Psychosocial Support & Intervention

  • Assess psychosocial needs, coping abilities, and support systems
  • Connect patients and families to community resources and financial assistance
  • Identify and address concerns such as abuse, neglect, and mental health needs
  • Advocate for patient autonomy and informed decision-making

Discharge Planning

  • Coordinate discharge plans for complex patients
  • Facilitate transitions to post-acute care and community services
  • Ensure compliance with payer and regulatory requirements

Patient & Family Support

  • Provide crisis intervention and support (e.g., abuse, domestic violence, end-of-life decisions)
  • Report suspected abuse to appropriate agencies
  • Assist with guardianship, advance directives, and care planning

Bereavement Support

  • Provide grief counseling and emotional support
  • Assist with grief support groups and community education initiatives

Compliance & Professional Development

  • Adhere to all legal, ethical, and regulatory standards
  • Participate in interdisciplinary collaboration and quality initiatives
  • Maintain ongoing professional development

Qualifications:

  • Education: Master's in Social Work or related field
  • Certifications:
    • Food Handler's Card
    • DPS Level 1 Fingerprint Clearance Card
  • Licensure: AZ driver's license (must be 21 years of age or older)
  • Experience: 3+ years social work experience in a healthcare setting

Work Requirements: Ability to meet post-discharge clinical & social needs; knowledge of clinical & social factors that may affect the patient's functional status at discharge. Ability to sit, stand, bend, and walk for entire scheduled shift. Expected duties have routine potential for exposure to blood, bodily fluids, or tissue.
