Care Manager (Bilingual - Spanish)

A leading healthcare organization is seeking a bilingual Care Manager to join their team!

The Care Manager is responsible for coordinating patient care, developing and updating personalized care plans, and ensuring patients receive appropriate support services. This role includes regular assessments, managing care plans, and conducting monthly telephonic visits and quarterly in-person visits. The Care Manager also connects patients to community resources and works with discharge planners to ensure seamless care transitions.

Pay Rate: $27.76/hr
Shift: Monday – Friday / 8:00AM – 5:00PM
Location: Los Angeles, CA (Watts)

Care Manager Job Requirements:

• 3+ years’ experience in Care/Case Management or a healthcare-related Bachelor’s Degree
• Bilingual (English/Spanish) and excellent communication skills
• Knowledge of and ability to work collaboratively with providers, support staff, social support services and other external entities related to care of patient
• Exhibit strong communication skills
• CPR certified (Adult, Child, Infant) by AHA or American Red Cross
