Bonus Eligibility: 3%-7% annually

Second Heart Homes, Inc. is a 501(c)(3) not-for-profit organization registered in the state of Florida dedicated to providing permanent supportive housing and wrap-around services for individuals with mental illness who are currently homeless or at-risk for homelessness.

Our mission is to revive the dignity and mental wellness of individuals experiencing homelessness through housing, support and love.

Our goal is to provide long-term solutions that empower our residents to revive their dignity, achieve stability and self-sufficiency, and enhance their quality of life so they may reach their potential as individuals and contributing members of the community.

We provide housing with daily supportive services that are tailored to each individual's unique situation and needs. This comprehensive approach helps our clients identify and reach their goals.

Position Summary

The Care Manager is responsible for the overall coordination of client services and daily support within Second Heart Homes residences. This role ensures that clients receive the services they need while maintaining a safe, clean, and supportive living environment.

This is not a traditional case management role. Second Heart Homes operates a unique, hands-on housing model that requires creativity, flexibility, and a deep commitment to serving individuals with mental illness. The Care Manager will champion the people we serve, think outside the box, and work collaboratively to help clients build stability in every area of life.

The Care Manager reports directly to the Director of Operations and works under regular supervision in accordance with agency policies and procedures.

Work Schedule

  • Five-day work week with rotating weekend coverage
  • Rare but possible after-hours emergency response
  • Occasional mandatory attendance at fundraising or organizational events

Key Responsibilities

  • Conduct daily visits to SHH homes and work with clients based on individualized plans
  • Enforce program rules and address behavioral concerns, including difficult conversations when necessary
  • Assist clients in developing and achieving life-skills goals
  • Coordinate services with community partners and benefit providers
  • Serve as client advocate and assist during crises and emergency situations
  • Maintain accurate case records and enter data into the HMIS system
  • Attend court hearings via video and speak on behalf of clients when applicable
  • Support cleanliness and organization of client homes; assist with light cleaning when needed
  • Transport clients to appointments as required
  • Purchase and deliver household supplies using SHH issued debit card
  • Collect monthly program fees and issue receipts
  • Develop and deliver client workshops
  • Maintain confidentiality and comply with HIPAA and organizational policies

Required Qualifications

  • High school diploma required; bachelor’s degree preferred (experience may substitute)
  • Valid driver’s license, proof of insurance, and reliable transportation (not public transit)
  • Computer proficiency, including Word, Excel, databases, and internet use
  • Ability to work independently and follow direction
  • Strong verbal and written communication skills
  • Ability to organize time, manage multiple tasks, and meet deadlines
  • Knowledge of community resources
  • Ability to maintain confidentiality and professional boundaries

Preferred Qualifications

  • At least two years working with individuals experiencing homelessness
  • At least two years working with individuals managing mental illness
  • Social services experience
  • CPR certification
  • Bilingual (a plus, not required)
  • Experience in fast-paced, high-stress environments (such as child protection or similar settings)

Physical & Work Requirements

  • Ability to lift at least 30 pounds
  • Ability to sit, stand, walk, and work in the field and at a desk for prolonged periods
  • Ability to work outdoors with clients when necessary
  • Ability to perform tasks requiring manual dexterity and coordination
  • Ability to work under stress and during irregular hours

Pay: From $45,000.00 per year
