Master Social Worker

Job Summary
Location Focus: Salt Lake and Utah counties. We are seeking a dedicated and compassionate Master Social Worker to join our dynamic healthcare team. In this role, you will provide expert clinical social work services to hospice patients and their families, ensuring comprehensive assessments, personalized care planning, and end-of-life supportive services.

Responsibilities

• Conduct thorough patient assessments to identify psychosocial needs, at the end-of-life and family dynamics.
• Develop and implement individualized care plans aligned with clinical goals and aligned with patient and family preferences.
• Collaborate with multidisciplinary teams to coordinate appropriate patient and family care, ensuring compliance with HIPAA regulations and medical documentation standards.
• Assist with difficult conversations during the end-of-life journey.
• Contribute to quality improvement initiatives within the organization.
• Maintain accurate medical records using electronic medical record (EMR) systems while adhering to confidentiality protocols under HIPAA guidelines.

Skills

• Experience in hospice care, geriatric populations preferred.
• Expertise in dealing with a patient and family at the end-of-life, intake procedures, diagnostic evaluation, case management.
• Familiarity with EMR systems and medical records management.
• Ability to work effectively with diverse populations including those with end-of-life diagnosis's , Alzheimer's disease, dementia, or other post-acute care needs.
