Job Overview
We are seeking a dedicated and compassionate Registered Nurse Care Manager to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine individual care needs.
- Develop, implement, and monitor personalized care plans that align with best practices in acute care, outpatient services, and post-acute care settings.
- Collaborate with multidisciplinary teams across hospital medicine, emergency medicine, behavioral health, geriatrics, pediatrics, and specialty cases to coordinate patient treatment strategies.
- Manage utilization review and case management processes to optimize resource use while maintaining high-quality patient care standards.
- Facilitate discharge planning by coordinating follow-up appointments, home health services, and outpatient therapies to ensure smooth patient transitions.
- Maintain accurate medical records in compliance with HIPAA regulations and policies; document all patient interactions thoroughly.
- Participate in diagnostic evaluations and medical record reviews to support clinical decision-making and improve care delivery efficiency.
Qualifications
- Valid registered nurse license with current state licensure required.
- Demonstrated experience in medicine, emergency settings, etc.
- Strong knowledge of infectious disease care, physiology principles, and acute care protocols.
- Experience with discharge planning, utilization management, and case management processes.
- Familiarity with medical terminology, diagnostic evaluation procedures, and medical records management.
- Excellent patient assessment skills combined with compassionate patient service orientation.
- Ability to work effectively in home care, inpatient and outpatient settings
- Knowledge of HIPAA compliance standards and confidentiality protocols.
- Strong organizational skills with the ability to handle multiple priorities efficiently in fast-paced healthcare settings.
