Job Summary

Work Schedule: M-F 8A-430P

Provides medical social services to patients and families having emotional, social, environmental, and financial problems under the clinical supervision and oversight of an MSW from a school of social work accredited by the CSWE.

Qualifications

Education Requirements

  • 4 year/Bachelor's Degree
  • Completes and maintains all competencies and trainings as required for role.

Experience

  • Home Care Social Work Experience, preferred

Certification and Licensure

  • Current Valid Driver's License
  • BLS Certification must be obtained within 60 days of hire date
