Social Worker – Woburn

Location: Woburn, MA

Job Summary:
HireCare Services – Massachusetts is seeking a compassionate and organized Social Worker to support residents and families in a skilled nursing/rehab setting at Woburn Rehabilitation and Nursing Center. As a Social Worker, you will focus on psychosocial assessments, discharge planning, care coordination, and helping residents thrive during their stay.

Responsibilities:

• Complete psychosocial assessments and contribute to individualized care plans
• Provide support/counseling to residents and families and assist with adjustment to the facility
• Lead and support discharge planning (coordination with hospitals, families, and community resources)
• Collaborate with nursing, therapy, and the interdisciplinary team
• Assist with admissions, transfers, and resident/family communication
• Ensure timely, accurate documentation and compliance with state/federal guidelines
• Participate in care plan meetings and follow up on assigned action items

Requirements:

• BSW required (MSW preferred)
• Experience in SNF/LTC/Rehab strongly preferred
• Strong communication and documentation skills
• Familiarity with discharge planning, community resources, and resident rights
• Organized, empathetic, and able to manage multiple priorities

Pay & Schedule:

• $40–$45/hour (based on experience)
• Schedule: Full-time
