Position Overview:
- Join our team as a Care Coordinator where you will work with the MSHO and MSC+ (Minnesota Senior Health Options and Minnesota Senior Care Plus, 65+ years of age) and the SNBC (Special Needs Basic Care, 18-64 years of age) population in a hybrid position. As a Care Coordinator, you will manage member caseloads within your assigned geographic area. In this position, you will work from home, but regularly travel to your local area to serve the needs of your members in the community.
Duties/responsibilities include but are not limited to:
- Coordinate Face to Face or remote visits/assessments based on member preference and Health Plan requirements
- Manage the Elderly Waiver
- Conduct annual assessments, including Personal Care Assistance (PCA) assessments for community members and customized living tools for members residing in an Assisted Living as needed
- Review current health care needs, identifying goals, and developing individualized member centered care plans
- Helping connect members with resources and necessary services
- Completing required documentation
- Coordinate with members interdisciplinary team
- Assist the Health Care Program Supervisor to assign caseloads, provide float coverage as needed, onboard new staff, and ensure the program policies and procedures are followed.
Qualifications:
Required
- Bachelor's Degree in social work, psychology, sociology or related field and at least three years of experience in health care or human services field
- Able to maintain professional boundaries & confidentiality
- Professional verbal and written communications skills
- Accept & adapt to changes
- Computer skills
- Be dependable
- Organized and attentive to detail
- Valid driver's license and a reliable, insured vehicle with a clean driving record
- Creative thinker & professionally solve problems effectively
- Take initiative, work independently with minimal supervision

Preferred
- RN or LSW (Registered Nurse or Licensed Social Worker)
- Completion of MNChoices Certified Assessor training
- Bi-lingual
- Experience working with state and federally funded programs (Medicare, Medicaid and other state programs)
- Experience working with the 65+ population

Wages:
- $55,000/year, starting wage based on experience
Frequently Asked Questions
Hours- Schedule is determined by staff and consumers. Traditional hours are 8-4:30 but are flexible. Staff can create their own schedule.
Mileage reimbursement- Mileage is reimbursed at .58 cents per mile

Evaluations - Evaluations occur every September for the year. All employees have monthly 1:1s with their supervisor to consistently evaluate performance. In Sept the evaluation is a monthly 1:1 but encompassing the whole year.
