Responsibilities

• Provides continuous supervision and case management services to assigned consumers on caseload

• Assists consumers with their activities of daily living skills, budgeting, and linkage to applicable benefits

• Provides consumers with medication education and observes their self-administration of medication

• Must adhere to the agency’s driving policy of transporting consumers in a safe and responsible manner

• Provides input to treatment and service plans as well as their implementation

• Must be able to document all relevant information to the consumers' electronic chart records

Qualifications

• BSW in social work, BA/BS in psychology, criminal justice, sociology, or a related field

• BA in an unrelated field with two years of previous experience

• AA in a related field with two years of previous experience

• Previous experience desired

• Valid Driver’s License
