Job Summary
We are seeking a dynamic and compassionate Client Care Coordinator to join our healthcare team. Responsibilities
- Act as the main liaison between patients, families, healthcare providers, and insurance companies to coordinate care effectively.
- Conduct intake assessments to gather comprehensive patient information and identify individual needs.
- Assist with discharge planning by coordinating services, scheduling follow-up appointments, and ensuring smooth transitions of care.
- Utilize electronic health record (EHR) systems and electronic medical records (EMR) systems to document interactions accurately and efficiently.
- Perform utilization management and review medical necessity for services in compliance with HIPAA regulations and insurance requirements.
- Support case management activities by developing personalized care plans, monitoring progress, and adjusting strategies as needed.
- Collaborate with multidisciplinary teams including social workers, nurses, physicians, and behavioral health specialists to optimize patient care pathways.
- Maintain up-to-date knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding standards, and hospital protocols across various specialties such as ICU, pediatrics, trauma centers, and hospice care.
- Ensure compliance with all regulatory standards including HIPAA privacy rules while managing sensitive patient information.
Experience
- Proven experience in critical care environments such as ICU or emergency medicine settings is highly preferred.
- Hospital experience with acute care or Level I/II trauma centers will be considered a strong asset.
- Background in nursing or social work with familiarity in discharge planning and case management practices.
- Knowledge of ICD-9/ICD-10 coding systems, CPT coding procedures, and utilization review processes.
- Experience working within EMR/EHR systems and managed care environments is essential.
- Familiarity with hospital medicine workflows including OR experience or PICU settings is advantageous.
- Strong understanding of healthcare regulations such as HIPAA and utilization management standards.
- Excellent communication skills with the ability to coordinate across multidisciplinary teams effectively.
