Home Health Staffing & Care Coordinator

We are a Medicare-certified home health agency in Sherman Oaks looking for an experienced Home Health Staffing & Care Coordinator to join our office team. This is a full-time, in-office role at the center of our daily operations — coordinating patient care, staffing our field clinicians, and keeping everything moving.

We are looking for someone who already understands the home health environment — the terminology, the workflows, the urgency — and is organized, reliable, and resourceful enough to hit the ground running.

WHAT YOU’LL DO

• Call patients and families to schedule visits, confirm care plans, and address concerns
• Contact physician offices to obtain orders, authorizations, and clinical updates
• Schedule and staff field clinicians: RNs, LVNs, CHHAs, and therapy staff (PT, OT, ST)
• Update and maintain our EMR system (Kinnser/WellSky) — training provided
• Track visit compliance and flag scheduling gaps to clinical leadership
• Support intake coordination, including insurance eligibility and benefit verification
• Handle general administrative and coordination tasks as needed

REQUIREMENTS

• Minimum 1 year of experience in a home health, hospice, or home care setting — required
• Strong communicator — you will be on the phone with patients, families, and doctors daily
• Highly organized with the ability to manage multiple priorities at once
• Comfortable with computers, email, and office software (Google Workspace or Microsoft 365)
• Able to learn new software independently — resourceful and self-directed
• Professional, empathetic phone manner

PREFERRED (NOT REQUIRED)

• Prior scheduling, staffing, or patient coordination experience
• Kinnser/WellSky or similar home health EMR experience
