Senior Field RN Care Manager Highlights
  • Monday–Friday schedule (8:30am – 5:00pm)
  • Field-based role conducting 2–4 UAS assessments daily
  • Must reside in Albany or Rensselaer County, NY
  • Permanent, direct hire opportunity
Senior Field RN Care Manager Responsibilities
  • Conduct UAS assessments and reassessments for LTSS members
  • Develop, implement, and manage individualized care plans
  • Coordinate care across providers, caregivers, and community resources
  • Monitor member progress and adjust care plans as needed
  • Educate members and families on services, benefits, and care options
  • Collaborate with interdisciplinary care teams to ensure quality outcomes
  • Maintain accurate documentation in compliance with state and federal guidelines
  • Identify high-risk members and support appropriate level-of-care transitions
Senior Field RN Care Manager Requirements
  • Active New York RN license
  • 4+ years of RN experience
  • UAS assessment experience required
  • Ability to perform field-based home/site visits
  • Strong care management and coordination experience
  • Valid driver’s license (personal transportation preferred)
Preferred Qualifications
  • Bachelor’s degree in Nursing (BSN)
  • Experience in Managed Care / MLTC / LTSS
  • Background working with high-acuity populations
  • Knowledge of community resources and care coordination
