Care Manager, LTSS - Story County

Experteer Overview As a field-based care coordinator, you support care management and coordination for high-need members across the continuum, working with a multidisciplinary team to deliver integrated, quality, and cost-effective member care. You will help members progress toward outcomes, contribute to strategy, and navigate LTSS and behavioral health integration in collaboration with providers and families. This remote role includes travel within Story County to conduct assessments and enable seamless care. You will be a key part of Molina’s mission to provide quality healthcare to people receiving government assistance. Compensation / Benefits • Complete comprehensive member assessments within regulated timelines, including in-person home visits • Facilitate waiver enrollment and disenrollment processes • Develop and implement care/waiver service plans with members, caregivers, and providers • Monitor and adjust care plans based on progress and interventions • Promote integration of services across LTSS, behavioral health, and community resources • Assess medical necessity and authorize waiver services • Evaluate benefits and advise on funding sources • Coordinate interdisciplinary care team meetings for service approvals • Use motivational interviewing to educate and motivate change • Identify barriers to care and provide coordinated support across psycho/social, financial, and medical needs • Identify critical incidents and develop prevention plans • Collaborate with licensed care managers/leadership as needed • Travel 25-40% of time depending on state/contract requirements Tasks • Health care experience (2+ years) including LTSS and care management experience (1+ year) or equivalent mix of education/experience • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) with active, unrestricted license if required by state • Bachelor’s degree in a health care-related field may be required in some states (dependent on state/contract) • Valid unrestricted driver’s license and reliable transportation for job-related travel • Knowledge of community resources • Ability to work across diverse settings with varying populations and personalities • Proactive, detail-oriented, self-motivated, and able to work independently • Strong communication (verbal/written) and time-management skills • Proficiency with Microsoft Office and navigating online portals/databases • Preferred: Certified Case Manager (CCM) and waiver services experience • Light to moderate ability to travel as required by contract • Demonstrated ability to build professional relationships • Ability to handle high-pressure situations calmly Key requirements • competitive benefits and compensation package
