Experteer Overview As an Inpatient LCSW & Care Manager, you will deliver patient-centered care management for at-risk inpatients, guiding assessment, planning, and discharge coordination. You will advocate for patients, address barriers to resources, and ensure regulatory compliance to maintain continuity of care. You’ll work within a team dedicated to compassionate, whole-person care and the mission to serve vulnerable communities.
