We are seeking a compassionate and experienced Master’s-level Social Worker (MSW) to join our Home Health team on a PRN basis. This is a fully remote position, offering flexible scheduling while making a meaningful impact in the lives of patients and families across Ohio. The ideal candidate is licensed in Ohio and brings strong assessment, coordination, and advocacy skills to support patients navigating complex health and social needs.
Key Responsibilities
- Provide psychosocial assessments for patients receiving home health services
- Develop individualized care plans addressing social, emotional, and environmental needs
- Coordinate community resources and services including financial assistance, transportation, housing, or counseling
- Collaborate with interdisciplinary care teams to support patient goals and care plans
- Offer crisis intervention and short-term counseling when necessary
- Document all interventions and patient communications in accordance with Medicare/Medicaid and agency standards
- Ensure patient confidentiality and uphold ethical standards of practice
- Participate in team meetings, case conferences, and ongoing education as needed
Requirements
- Master's degree in Social Work (MSW) from an accredited institution.
- Valid state licensure as a Social Worker is required.
- Strong understanding of behavioral health principles and practices.
- Excellent communication skills, both verbal and written.
- Experience in case management is preferred but not mandatory.
- Ability to work collaboratively within a multidisciplinary team.
