Registered Nurse Care Manager

Duties

• Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
• Develop and implement individualized care plans that align with best practices in hospital medicine, outpatient clinics, or post-acute settings.
• Collaborate with physicians, specialists, and other healthcare professionals to coordinate inpatient and outpatient care strategies.
• Manage utilization review processes by evaluating diagnostic evaluations and ensuring appropriate hospital or outpatient services are provided efficiently.
• Lead discharge planning efforts by coordinating post-acute care services, home health resources, and community support systems.
• Maintain accurate medical records in compliance with HIPAA regulations while documenting patient interactions and care plans thoroughly.
• Engage in patient service activities such as intake assessments, case management, and patient education to promote optimal health outcomes.

Requirements

• Valid nursing license with current registration in the applicable state or jurisdiction.
• Critical care experience with a strong background in ICU or emergency medicine settings.
• Experience working within Level I or Level II trauma centers, demonstrating expertise in trauma and infectious disease care.
• Knowledge of hospital medicine, medical-surgical units, pediatrics (PICU), geriatrics, behavioral health, and outpatient clinics.
• Proficiency in medical terminology, physiology knowledge, diagnostic evaluation, and medical records management.
• Skilled in patient assessment techniques including vital signs monitoring and clinical observations.
• Familiarity with HIPAA compliance standards and healthcare regulations related to inpatient and outpatient services.
• Strong organizational skills for discharge planning, utilization management, intake processes, and case management activities.
• Ability to adapt quickly to diverse clinical environments such as emergency rooms, inpatient units, or outpatient clinics while maintaining high-quality patient care standards.
