This position is an excellent opportunity for LVNs seeking to expand their impact within a respected, mission‑driven health organization.Overview: Our client is seeking a compassionate and dedicated Care Manager, LVN to join a mission‑driven team focused on healing and inspiring the human spirit. This role is ideal for Licensed Vocational Nurses who want to move beyond routine clinical work and make a meaningful impact through coordinated care, advocacy, and collaboration. Working under departmental leadership, the Care Manager, LVN partners closely with providers, hospitals, IPA medical groups, community agencies, caregivers, and Members. The LVN supports the care management process alongside RN Care Managers and the interdisciplinary care team (ICT), ensuring Members receive high‑quality, coordinated services aligned with program goals, including HEDIS, CAHPS, and NCQA standards.Key Responsibilities:-Serve as a resource for IEHP Members and Providers regarding care management and care coordination.-Screen Members for care management needs using approved tools (e.g., Health Risk Assessment) and communicate findings to RN Care Managers.-Assist in developing, updating, and reviewing Individualized Care Plans (ICP) with RN Care Managers and ICT members.-Provide Member education based on approved care plans and materials.-Communicate unexpected clinical or care‑related findings to RN Care Managers and ICT.-Advocate for Members and act as a liaison between Members, caregivers, and the interdisciplinary team.-Facilitate referrals and support services, including:-Assist Members with immediate needs related to access, referrals, and authorizations.-Utilize evidence‑based communication techniques (e.g., Motivational Interviewing) during Member interactions.-Build and maintain strong working relationships with Providers through in‑person, phone, email, and fax communication.-Participate in quality improvement initiatives and support efforts to close HEDIS gaps.-Coordinate medical and behavioral health access with PCPs, specialists, and ancillary providers.-Perform additional duties as assigned by supervisors or RN Care Managers.
