Director of Social Work

A highly rated skilled nursing facility in Canandaigua, NY is seeking a Director of Social Work to guide and elevate our Social Services department. This is a key leadership role for someone passionate about advocacy, resident well-being, and team development—within a supportive, non-mandated environment.

Compensation: $90,000–$120,000/year (DOE)

What You’ll Do:

• Direct and oversee all Social Work services across the facility
• Lead discharge planning, care coordination, and psychosocial support initiatives
• Partner with interdisciplinary leadership to drive quality outcomes
• Serve as a key resource for residents, families, and community partners
• Ensure compliance with all state, federal, and CMS regulations
• Mentor and develop Social Services staff

What We’re Looking For:

• Bachelor’s Degree in Social Work or related field (MSW preferred)
• Prior leadership experience in a SNF or healthcare setting
• Strong knowledge of Medicaid/Medicare and discharge planning
• Proven ability to lead with empathy, organization, and accountability
