Responsibilities:
- Provide crisis management and intervention services to patients and their families
- Conduct research and stay updated on best practices in medical social work
- Develop and implement care plans for patients with PTSD, senior care needs, special education requirements, pediatrics, autism, and other specific conditions
- Collaborate with healthcare professionals to ensure comprehensive care for patients
- Conduct assessments and evaluations to determine patient needs and develop appropriate interventions
- Provide counseling and support to patients and families dealing with medical challenges
- Utilize ICD-9 coding system for accurate documentation and billing purposes
- Advocate for patients' rights and access to necessary resources
- Utilize motivational interviewing techniques to promote behavior change and improve patient outcomes
Qualifications:
- Master's degree in Social Work or related field
- Valid licensure as a Medical Social Worker
- Experience working with diverse populations, including seniors, children, individuals with disabilities, and individuals with mental health challenges
- Knowledge of medical terminology and healthcare systems
- Strong communication and interpersonal skills
- Ability to work independently as well as part of a multidisciplinary team
- Strong organizational skills and ability to manage multiple cases simultaneously
