Job Overview
Description
Coordinates incoming office referrals and verifies insurance coverage. Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.
Day Shift 5 x 8s, M - F 8am to 5pm. MUST have LVN certification. At least 1 year of experience. This will be a clinical intake coordinator for home health processing and scheduling referrals. No Call.

LVN certification
Must have Clinical Intake Coordinator experience
Pay: $30.96 - $31.00 per hour
Expected hours: 40.0 per week
