Social Services Clinician (LCSW / LPC / LMFT)

Job Description:
We are seeking a skilled Social Services Clinician to join a multidisciplinary team in a Level IV Critical Access Hospital in Southeast Alaska. In this role, you will provide discharge planning, clinical assessment, brief therapy, and care coordination for patients across the lifespan. You will collaborate with healthcare providers and community resources to ensure seamless transitions of care while supporting patient well-being and family needs.
Responsibilities:
- Conduct patient screenings, assessments, and treatment planning
- Lead discharge planning and coordinate transitions of care
- Provide brief therapy and support for mental health and substance use needs
- Collaborate with interdisciplinary teams and community partners
- Provide patient advocacy, care coordination, and resource linkage
- Maintain accurate and compliant clinical documentation
Requirements:
Master’s Degree in Social Work, Counseling, Psychology, or related field
Licensed or in process of LCSW, LPC, or LMFT
Valid Driver’s License (or ability to obtain within 6 months)
Must be 21+ years of age
Experience in discharge planning, case management, or behavioral health preferred
Pay: $81,000.00 - $104,000.00 per year
