Position Overview
The Intake Coordinator plays a vital role in the client acquisition process by acting as the first point of contact for potential clients and their families. This energetic and proactive individual is responsible for identifying the needs of prospective clients, nurturing leads, and ensuring a smooth transition into care services. The Intake Coordinator excels in managing inbound inquiries and conducting outbound outreach through phone, email, and digital channels. They also take charge of conducting virtual assessments to evaluate client needs and provide initial recommendations for care. With a focus on client engagement, they gather essential information, schedule in-person or virtual assessments, and facilitate consultations, ensuring that the organization can provide the appropriate level of care based on the client’s specific needs. Their collaboration with branch locations and on-call teams is key to streamlining operations and contributing to the overall success of the company. Three years of experience in home care or healthcare is preferred, as this role is essential in managing the early stages of the client lifecycle and building lasting relationships.

Essential Duties And Responsibilities
  • Handle and manage initial intake calls, converting qualified leads into scheduled assessments, including virtual assessments when appropriate.
  • Proactively generate new sales opportunities through lead-generation techniques such as email outreach, cold calls, and social media engagement.
  • Clearly articulate Care Indeed's values, services, and offerings to prospective clients while empathetically listening to and understanding their unique needs, ensuring the right care solution is recommended.
  • Meet and exceed intake sales targets, conversion quotas, and other performance metrics.
  • Track, manage, and report the status of all leads, prospects, and clients through the company's internal systems, ensuring timely follow-up and communication.
  • Maintain data accuracy, consistency, and timeliness across all systems to ensure operational efficiency.
  • Serve as a key liaison between new clients, assessors, care management teams, and direct care supervisors, ensuring clear communication and smooth transitions.
  • Collaborate with accounting, direct care supervisors, and other internal teams to provide a seamless client onboarding experience and address any administrative needs.
  • Guide clients through every step of the intake process, from the initial inquiry to the commencement of care, ensuring they feel supported and informed.
  • Communicate regularly with remote and on-call teams to support a seamless transition from the intake process to ongoing client care management.
  • Prepared and presented sales analytics reports on a weekly, monthly, and quarterly basis, tracking lead generation, conversion rates, and other key performance metrics.
  • Provide ongoing support to assessors and ensure a smooth transition for clients from the sales process to care coordination.
  • Perform additional duties as assigned, contributing to the overall goals and objectives of the company.

Job Conditions

  • It is primarily a desk job that essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication

Qualifications
  • A Bachelor’s degree is required - a background in social work, psychology, counseling, rehabilitation, gerontology, nursing, or sociology
  • At least one year of experience or training in health care/social services.
  • Sales experience preferred.
  • Exceptional written and verbal communication skills.
  • Proficiency in assessing client needs, meeting service quality standards, and evaluating client and employee satisfaction.
  • Capability to thrive in a fast-paced environment.
  • Unwavering commitment to providing the highest quality care and customer service.
  • Proven track record of process management with meticulous attention to detail.
  • Sensitivity, patience, and compassion when interacting with prospects.
  • A positive, solution-oriented mindset.
  • Flexibility and availability to work hours beyond the standard workweek.

Work Hours
Monday-Friday 9:00 AM to 5:30 PM
