About Exodus Project

Exodus Project provides support and services to individuals returning to the community after incarceration, including housing navigation, employment support, counseling, case management, and behavioral health connections.

Our team works to ensure individuals leaving incarceration have access to support, resources, and stabilization during critical moments of transition.

Position Summary

The Post-Release Care Coordinator provides after-hours and weekend support for individuals recently released from jail or prison. This role focuses on intake, crisis response, service navigation, and connecting individuals with resources until daytime services resume.

This position requires someone who can remain calm under pressure, communicate clearly, and support individuals during moments of instability.

Key Responsibilities

  • Respond to after-hours and weekend client needs
  • Provide intake and onboarding support for individuals recently released from incarceration
  • Offer crisis response and de-escalation support
  • Assist individuals with short-term stabilization and service navigation
  • Transport clients when appropriate, including jail pick-ups when authorized
  • Connect clients with housing, services, and community resources
  • Document services and client interactions in required systems
  • Communicate with daytime staff to ensure continuity of care
  • Maintain professional boundaries while practicing trauma-informed care

Qualifications

Required

  • High school diploma or equivalent
  • Valid driver’s license

Preferred

  • Experience supporting individuals in crisis or transition
  • Experience in social services, community-based programs, or reentry services
  • Comfort learning and using documentation systems and program software

Skills and Abilities

  • Strong boundary setting and professionalism
  • Crisis response and de-escalation skills
  • Trauma-informed and culturally humble engagement
  • Strong documentation and time management skills
  • Ability to work independently during overnight or weekend shifts

Available Positions and Schedules

1.0 FTE
Graveyard shift (4 × 10-hour schedule)
12:00 AM – 10:30 AM

0.6 FTE
Friday, Saturday, Sunday
12:00 AM – 8:30 AM

0.4 FTE
Saturday and Sunday
4:00 PM – 12:30 AM

Compensation

$23 – $25 per hour
