Social Worker (MSW / LCSW / LMSW)
Location: Turlock,CA
Shift: Day Shift (varies by facility)

Key Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families
• Develop and implement individualized care and discharge plans
• Provide counseling and emotional support to patients and families
• Coordinate with physicians, nurses, and interdisciplinary teams
• Assist with discharge planning, placement, and community resource referrals
• Advocate for patients’ needs and ensure continuity of care

Requirements

• Master’s Degree in Social Work (MSW) required
• Active State License (LCSW / LMSW or equivalent)
• Minimum 1–2 years of healthcare or hospital experience preferred
• Strong communication, counseling, and case management skills
• Ability to work in fast-paced healthcare environments

Job Types: Full-time, Contract

Pay: $37.29 - $48.00 per hour

Expected hours: 40 per week
