Setting: FQHC / Community Health Outpatient Clinic
Schedule: Monday - Friday (Day Shift)

Job Summary:

We are seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive mental and behavioral health services within an Integrated Behavioral Health (IBH) model. This role delivers patient-centered care through mental health and substance use assessments, short-term evidence-based therapy, crisis intervention, and care coordination in collaboration with multidisciplinary teams.

Key Responsibilities:

• Conduct comprehensive mental health and substance use assessments.

• Perform integrated behavioral health screenings in a primary care setting.

• Provide short-term, solution-focused therapy using evidence-based approaches (CBT, Motivational Interviewing, Trauma-Informed Care).

• Develop and implement individualized treatment plans.

• Provide individual and group therapy.

• Conduct crisis intervention and risk assessments.

• Coordinate referrals to specialty mental health, substance use treatment, and community resources.

• Collaborate with care teams within the IBH model.

• Maintain accurate, compliant clinical documentation.

• Deliver culturally competent care and educate patients/families.

Qualifications:

• Master's degree in Social Work (MSW) from a CSWE-accredited institution.

• Active California LCSW license.

• Minimum 2-3 years of clinical experience required.

• Experience in integrated/community healthcare preferred.

• Strong background in psychosocial assessments, treatment planning, crisis intervention, and substance misuse treatment.

• Familiarity with DSM-5 criteria and behavioral health insurance guidelines.

• Experience working with adolescents/minors preferred.
