Case Management Coordinator supports coordinated care for an assigned patient population across the continuum, ensuring appropriate resource use, regulatory compliance, and effective discharge planning through integrated case management and utilization review. Assists with assessments, care planning, and evaluation to promote quality, continuity, patient satisfaction, and cost effective outcomes. Completes daily CMS Quality reporting; handles PHI for referrals; maintains HLOC files for LTAC/SNF; and assists case managers and social workers with discharge planning tasks. Coordinates in services, supports office operations by maintaining supplies and equipment, and performs additional duties to ensure efficient case management workflows.
Qualifications:
Education: Graduate of an accredited program for licensed practical nurses.
Licenses/Certification: Current Licensed Practical Nurse, LPN License issued in the state of facility location.
Experience: LPN with a minimum of three years of clinical experience, including work in admissions or clinical areas. Knowledgeable in regulatory and compliance requirements, discharge planning standards, and hospital policies. Applies professional standards of practice with strong organizational and analytical skills. Proficient in job related computer systems and software. Case Management experience preferred.
