If Yes, How You Will Contribute to the Team:

As a Care Coordinator, you will serve as a central liaison between residents, families, physicians, pharmacies, home health agencies, and hospice providers to ensure continuity and quality of care. Responsibilities include communicating with residents’ physicians to obtain and clarify medical orders, accurately entering and revising physician orders, coordinating medication services with contracted pharmacies, and following up on home health and hospice referrals and documentation.

You will maintain accurate and timely resident records by scanning, uploading, and organizing documentation within the electronic health record system. Additionally, you will support the Director of Nursing with clerical, compliance, and regulatory documentation to ensure adherence to state and facility standards.

Must Have:

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in health sciences, healthcare administration, or related field preferred.
  • Experience in assisted living, senior care, or healthcare setting.
  • Strong organizational and documentation skills with the ability to manage multiple residents and care plans.
  • Knowledge of resident assessments, service plans, and state regulations related to assisted living.
  • Ability to communicate effectively with residents, families, physicians, and interdisciplinary team members.
  • Proficiency in electronic health records and basic computer systems.

Nice to Have:

  • Previous experience as a Care Coordinator, Resident Care assistant, or similar role in assisted living or memory care.
  • Familiarity with Title 22 regulations and compliance standards in California.
  • Bilingual abilities (especially English/Spanish) to support resident and family communication.
