Job Summary
Position Summary
The Discharge Planner works under the director of the RN Case Manager and/or /Social Worker to complete referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree.
Responsibilities
Responsibilities include the following activities:
- Arranges post-discharge services, creates and follows-up on electronic referrals using the Tenet Case Management system,
- Reviews patient choice letters with patients/families for required signatures,
- Provides follow up Important Message to Medicare patients prior to discharge,
- Communicates with patient, families and other members of the care team, completes tasks assigned by Case Manager and Social Work staff,
- Make copies, send faxes, and completes phone calls,
- Completes process reviews or audits as requested.
- Completes established competencies for the position within designated introductory period.
- Other related duties as assigned.
INIMUM EDUCATION:
High School diploma or equivalent
PREFERRED EDUCATION: Associate’s or Bachelor’s Degree from an accredited college
MINIMUM EXPERIENCE: 6 months in health care environment. Possesses excellent organizational, verbal/written communication skills, problems solving and computer literacy skills. Data Analytics skills.
PREFERRED EXPERIENCE: Acute hospital experience
REQUIRED CERTIFICATIONS/LICENSURE: NA
PREFERRED CERTIFICATIONS/LICENSURE: Paramedic, EMT or Nursing Assistant certifications.
REQUIRED COURSES/ COMPLETIONS (e.g., CPR):
- Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire
- Non-Violent Crisis Intervention (NVCI) within 60 days of hire.
