Position Overview
The Care Coordinator is responsible for ensuring that individuals receiving behavioral health services are supported with high-quality care coordination, person-centered planning, and ongoing monitoring to promote positive clinical outcomes.
This role serves as a key point of contact for clients, families, and interdisciplinary team members, ensuring that services are delivered according to regulatory standards and agency expectations.
Key Responsibilities:
Client Intake & Assessment
- Conduct initial intake screenings, eligibility assessments, and service needs evaluations for new referrals.
- Collect and review clinical, psychosocial, and demographic information required to initiate services.
- Develop service recommendations in alignment with Medicaid, state, and agency criteria.
Care Coordination
- Serve as the primary liaison between clients, families, caregivers, and service providers.
- Coordinate internal and external services, including therapy, community support, case management, and crisis intervention.
- Ensure timely referrals and follow-up appointments to support continuity of care.
Supervision & Monitoring Visits
- Complete required face-to-face and virtual supervision visits with clients and paraprofessional staff (e.g., Behavioral Technicians).
- Observe service delivery, provide coaching, and document supervisory oversight in accordance with state requirements.
- Monitor progress toward goals and ensure interventions remain consistent with treatment plans.
Documentation & Compliance
- Maintain accurate, thorough, and timely documentation in the electronic health record system.
- Prepare treatment plans, progress notes, assessments, and supervisory reports in compliance with regulatory and agency standards.
- Ensure that services meet Medicaid, state licensing, and accreditation guidelines.
Collaboration & Team Support
- Participate in interdisciplinary team meetings, clinical staffing, and case consultations.
- Communicate regularly with therapists, paraprofessionals, and administrative staff to support coordinated service delivery.
- Provide guidance, mentoring, and oversight to direct care staff as needed.
Client Advocacy
- Support clients and families in navigating behavioral health systems, community resources, and support services.
- Advocate for client rights, access to care, and culturally responsive practices.
- Assist clients in overcoming barriers that may impact treatment engagement or progress.
Crisis Support
- Respond appropriately to urgent behavioral health needs and coordinate crisis intervention services when necessary.
- Ensure appropriate safety planning and follow-up for individuals experiencing crises.
Qualifications
- Experience in home care, behavioral health, or a related healthcare setting required.
- Must meet state criteria for Qualified Professional (QP), typically including:
- Bachelor’s degree (or higher) in a human services field and relevant behavioral health experience; or
- Licensed/credentialed professional meeting QP requirements per state guidelines.
- Strong understanding of behavioral health principles, Medicaid service definitions, and service delivery standards.
- Familiarity with local LME/MCO (Local Management Entity/Managed Care Organization) processes.
- Excellent communication, documentation, and organizational skills.
- Ability to work independently, manage time effectively, and maintain professionalism in diverse environments.
- Valid driver’s license and reliable transportation (if community-based services are required).
Work Environment
- Combination of office, client homes, community settings, and virtual visits.
- May require flexible scheduling to meet client and program needs.
