Position Overview:
We are seeking a dedicated and compassionate Care Coordinator for Psychiatric Patients to join our team. The ideal candidate will have a strong background in mental health care and be able to work closely with psychiatric patients to manage their care plans, appointments, and overall mental health treatment. This role requires a highly organized, empathetic individual with excellent communication skills to ensure optimal patient outcomes.
Responsibilities:
- Coordinate and manage care for psychiatric patients, ensuring they receive comprehensive services and support.
- Develop individualized care plans based on assessments of patient needs, including medical, psychological, and social factors.
- Work closely with mental health professionals (psychiatrists, therapists, social workers) to ensure continuity of care and treatment.
- Schedule appointments, track patient progress, and follow up to ensure compliance with treatment plans.
- Provide resources, support, and guidance to patients and their families, helping them navigate mental health services.
- Maintain detailed and accurate patient records, ensuring confidentiality and compliance with HIPAA regulations.
- Monitor patients' mental health status, and coordinate emergency interventions if necessary.
- Act as a liaison between patients, their families, and healthcare providers to address any concerns or needs.
- Stay current on mental health trends, treatments, and community resources.
Qualifications:
- Bachelor's degree in Nursing, Social Work, Psychology, or a related field (or equivalent experience).
- Previous experience in psychiatric care or working with mental health patients preferred.
- Strong understanding of mental health conditions, treatments, and care strategies.
- Excellent organizational skills and the ability to manage multiple patient cases at once.
- Strong communication and interpersonal skills, with the ability to interact with patients, families, and healthcare providers.
- Knowledge of HIPAA and confidentiality requirements.
- Ability to work independently and as part of a collaborative team.
- Compassionate, empathetic, and patient-focused.
Job Type: Full-time
Pay: $21.00 - $23.00 per year
