- Implements an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs.
- Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients’ aftercare needs
- Provides assistance and total support to the Clinical Director and Social Services team to enhance the social functioning of all patients

Qualifications
- Education: Bachelor’s Degree in social work or related field.
- Experience: Experience in inpatient or outpatient mental health setting preferred.
- Hours: Determined by the Director of Clinical services according to the needs of the facility
