Implements an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient’s aftercare needs.
Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients’ aftercare needs
Provides assistance and total support to the Clinical Director and Social Services team to enhance the social functioning of all patients

Qualifications

Education: Bachelor’s Degree in social work or related field.
Experience: Experience in inpatient or outpatient mental health setting preferred.
Hours: Determined by the Director of Clinical services according to the needs of the facility
