Join us in making a difference in the lives of those we serve, while embracing a culture of compassion, respect, and excellence.What You'll Do:Assess patient and family emotional, social, financial, and environmental needs (coping, safety, caregiver strain, resources, advance care planning readiness).Care coordination & resource navigation - connecting patients/families with community resources and coordinating with the interdisciplinary team.Participate in care plan development and case conferences.Serve as liaison between patients or families/caregivers and community agencies.What You Bring:Current state licensure, or eligibility for licensure as requiredStrong skills in psychosocial assessment, counseling, and care coordination.
