Join our dedicated team as a Community Stabilization Counselor - QMHP/QMHP-T, where you will play a vital role in providing comprehensive mental health support and stabilization services to individuals and families facing behavioral health challenges. Duties
- Providing individualized, outcome-focused care
- Implementing crisis intervention strategies and crisis management protocols to ensure immediate safety and stabilization during mental health emergencies.
- Strong proficiency in crisis intervention strategies and crisis management to effectively de-escalate situations and ensure client safety.
- Utilize effective de-escalation techniques to prevent further symptom escalation or disruptions in the individual’s current living environment.
- Refer and link individuals to all medically necessary behavioral health services and supports across the continuum of care.
- Coordinate and collaborate with all behavioral health providers involved in the individual’s care throughout service delivery.
- Coordinating with service providers, insurance companies, and family members to promote stabilization and reduce risk of hospitalization
- Provide services in a recovery-oriented, trauma-informed, culturally congruent, and developmentally appropriate manner, integrating Zero Suicide / Suicide Safer Care principles.
- Provide discharge planning services, including engagement and rapport-building; referring and connecting with appropriate services including mental health, substance use, medical, and natural supports; and assist clients in applying for benefits
- Write descriptive, concise, and clinically appropriate progress notes aligned with the individual’s defined goals and objectives.
- Complete all required documentation accurately and within required timeframes.
- Attend all assigned meetings, trainings, and supervision as required.

- Demonstrating strong verbal and written communication skills
Pay: $28.00 - $32.00 per hour
