The Intake Coordinator coordinates the intake process for the Supportive Services for Veteran Families (SSVF) program. Individual will conduct phone screens with potential program participants to determine eligibility. Intake Coordinator will also lead targeted outreach efforts to engage the veterans who are homeless or imminently at risk of becoming homeless and connects them to housing and training interventions. Person will be responsible for educates veteran service providers and stakeholders on SSVF eligibility, services and referral processes. This position will support both staff and clients across North Carolina.

Classification – Full-time salary position with flexible hours, including occasional evenings and weekends.

Duties / Responsibilities:
- Supports the SSVF Director and Senior Staff with administrative duties including scheduling, meeting logistics, packet creation and upkeep, copying, filing, answering phones, and greeting visitors;
- Communicates em-pathetically and effectively with potential and participating Veterans and supporting providers;
- Gathers data and generates reports for VSC programs as directed;
- Participates in all appropriate staff, supervision, and training meetings;
- Conducts 18 month follow up with Veterans to track long term housing sustainability;
- Coordinates and schedules screenings for SSVF via telephone, or in person at VSC as needed;
- Utilizes NCServes Unite Us technology platform for accurate referrals across region;.
- Conducts Integrated Assessments as well as any necessary sub-assessments for HVRP and PATH to determine eligibility for programs;
- Maintains information on available public and private resources and diverts individuals to these resources when appropriate;
- Collects and maintains accurate and detailed records of intake process to include outcomes, staffing, and/or ineligibility; and
- Assumes other duties as assigned by the VSC Director and Senior Staff.
Skills / Competencies:
- Minimum of 2 years working with at-risk populations; experience with Veterans preferred
- Experience, education and/or training in social work, counseling, case management, psychology or other related human service field and knowledge of area resources for homeless individuals preferred
- Excellent communication skills, both verbal and written
- Highly proficient in MS Excel, MS PowerPoint and MS Word
- Strong organizational skills and attention to detail
- Ability to work both as a member of a team and independently as needed
Qualifications:
- Associates Degree in Human Services or relevant experience; bachelor’s degree in human services or equivalent experience preferred
Relationships / Working Environment:
- The Intake Coordinator / Administrative Assistant reports to the SSVF Assistant Director and supports Senior Staff. The position interacts frequently with VSC staff, Veteran participants, and Community Providers.
- The position works in an office setting and requires occasional local travel.
- Physical Demands: Must be ambulatory; must be able to operate a vehicle; must have visual/hearing acuity. Must be able to write legibly. Must have sufficient manual dexterity to write and use a computer and other typical office equipment on a regular basis. Must possess dexterity, stand, sit, walk, lift and carry twenty pounds for short distances.
- Mental Demands: Must have verbal ability; must be able to comprehend instructions; comprehend/ interpret charts, diagrams, prescriptions, and possess inductive and deductive reasoning.
