- $100 per case, paid bi-weekly (covers all aspects of case management)
- Start with 3–5 cases, with opportunity to increase
- $50 per in-person assessment
Schedule
- Flexible scheduling based on caseload and availability
- Occasional in-home visits required
Job Summary
The Registered Nurse Care Coordinator serves as the primary clinical point of contact for clients and families, ensuring high-quality, coordinated care. This role is responsible for creating, maintaining, and reassessing individualised care plans while supporting caregivers and maintaining compliance with agency policies and regulations.
Key Responsibilities
- Serve as the primary RN contact for clients and their representatives
- Communicate via phone, email, and Family Portal regarding care questions or changes
- Coordinate client schedules with the scheduling department
- Communicate care plan details to caregivers, including prior to their first shift
- Coordinate care among all parties involved
- Conduct 90-day care plan reviews and nursing reassessments on time
- Supervise CNA/GNA staff as required
- Observe, assess, and document client symptoms and progress
- Document all client and caregiver-related events in ADLware
- Notify appropriate HIPAA representatives of changes in client condition
- Maintain accurate and timely patient records
- Follow infection control policies in and out of the office
- Provide in-home patient care per agency policy and procedure
- Participate in quality improvement initiatives
- Assist with new CNA/GNA orientation and ongoing training as needed
Job Requirements
- Current, active Registered Nurse (RN) license
- Minimum of 2 years of nursing experience
- Strong written and verbal communication skills
- Excellent documentation and organisational skills
- Ability to work collaboratively with clients, families, caregivers, and staff
