- Bachelor’s degree in the field of Social Work/Psychology/Gerontology or related field with strong writing and verbal communication skills
- A minimum of two years of experience is required working with geriatric residents in Long Term Care setting
- Knowledge of Title 22 Regulations and case management
- Ability to administer Depression Scale and Cognitive Testing
Residential Social Services shall plan and coordinate social services for the residents needing support in Independent, Assisted Living, and Memory Care units; provide educational programs for all Casa staff in effectively dealing with people with cognitive impairments; establish and facilitate a comprehensive support (group and individual) program for all residents with appropriate follow up.
Essential Job Duties
- Develops and implements a case management program for providing psychosocial support to all residents
- Assist residents in transition between the various levels of care as necessary and provides support for families to help them deal with these transitions.
- Provide support and resources on topics such as grief, loss, caregiving, hospice, and concerns related to aging. Maintains current resource list.
- Provides input for departmental operation and capital budgets, including possible revenue generated from counseling programs and case management.
- Consults with the Director of Resident Services, Director of Residential Continuing Care Director of Activities, and Administration to develop a holistic program to meet the needs of residents, on their individual levels, to enhance the quality of life.
- Coordinates regular, ongoing educational sessions for residents on age-related topics.
- Assumes a proactive role in resolving resident issues & concerns related to assigned areas of responsibility. Includes Medicare, and Long Term Care Insurance. Available to meet and support residents with various requests and any other duties as assigned.
- Provides input to RHAT (Resident Health Assessment Team) through personal observations and those of staff regarding changes in resident condition and develop an action plan.
- Assesses residents’ cognitive, functional, and psychosocial abilities on an annual basis.
- Serves as a liaison for residents in skilled nursing.
$28.50 - $32.00
