Job description

Alamo Health Management of Santa Clarita, provides transitional housing and resident centered care services for clients suffering from mental health, addiction, and/or dual diagnosis. The ideal candidate wants to help break the cycle of clients moving from crisis centers to housing instability and back to crisis centers.

We are looking for a Client Care Coordinator to join our team in our Santa Clarita community. The Client Care Coordinator provides direct supervision of caregiving within our Behavioral health community. Ensures residents are treated with respect and dignity, recognizing individual needs, and encouraging independence. Fostering a homelike atmosphere throughout the community.

Required Availability:

Tuesday - Saturday 8:30am - 5pm

Position Description:

  • Responsible for screening, hiring, training, and supervising all care staff and scheduling and managing care staff call offs.
  • Monitors staff in the administration of medication, including frequents audits of medication sheets and that medications are properly stored, medication carts/cupboards are cleaned, and medication room is organized according to standards.
  • Participates in the maintenance of resident records including review of documentation performed by resident assistants, monthly MAR (Medical Administration Record) reviews, and responsible for all local, state, and federal rules and regulations as they apply to services delivered in the Specified Care area.
  • Assists in organizing and developing service plan reviews as required, with appropriate resident care team members.
  • Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through.
  • Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions that assist others beyond job responsibilities and has a positive impact on co-workers, residents, and visitors. Has working knowledge of the organization.
  • Perform other duties as assigned or needed.
  • Shares on call duties as required.

Education & Experience:

  • High School Diploma or equivalent required.
  • Associate or Bachelor’s Degree strongly preferred in a field related to the position.
  • Must have proven management experience and ability to manage staff.
  • Minimum three years’ experience in assisted living preferred.
  • LVN, RN, or experience Med Tech supervisor preferred.

  • Mental Health or Behavioral Health: 1 year (Preferred)
  • Med Tech: 3 years (Preferred)

  • RN License (Preferred)
