- Conducting routine rounds throughout the Health Center to monitor resident well‑being, assess quality of care, and confirm that staffing patterns align with current resident needs and acuity levels.
- Ensuring the delivery of comprehensive and appropriate nursing services consistent with resident care plans, regulatory standards, and evidence‑based practice.
- Serving as a primary point of contact for residents, families, and staff, providing guidance, addressing concerns, and fostering a supportive and collaborative environment.
- Overseeing admissions, transfers, and discharges, coordinating closely with partner hospitals and teams across Assisted Living, Memory Support, and Independent Living for seamless transitions of care.
- Leading, supervising, and developing nursing and department leadership teams, including responsibility for hiring, onboarding, scheduling, coaching, performance evaluations, and addressing staff feedback and concerns.
- Active California Registered Nurse (RN) license and graduation from an accredited nursing program.
- Current CPR certification.
- Bachelor’s degree in Nursing required; Master’s degree preferred, or a comparable blend of education and relevant professional experience.
- 3–5 years of progressive nursing leadership experience, ideally in a Skilled Nursing Facility or related care environment.
- Strong clinical assessment and critical‑thinking abilities, with a keen eye for detail and commitment to best‑practice standards.
- Proficiency with electronic health records (EHRs) and comfort navigating computer‑based documentation, communication platforms, and reporting tools.
