Position Overview
The Certified Social Worker will provide supportive services to residents and families, assist with admissions and discharges, advocate on behalf of resident needs, and ensure regulatory compliance in all social services functions. This role is essential to fostering a warm, welcoming, and supportive environment within our skilled nursing facility.
Responsibilities
- Conduct assessments and develop individualized care plans
- Support residents and families through psychosocial needs, transitions, and care conferences
- Collaborate with an interdisciplinary team to ensure quality care
- Address resident rights, concerns, and quality-of-life issues
- Assist with discharge planning, placement, and community resource coordination
- Maintain accurate documentation and ensure regulatory compliance
Qualifications
- Certification as a Social Worker (required)
- Experience in a skilled nursing or healthcare setting preferred
- Strong communication, empathy, and problem‑solving skills
- Ability to build trust and rapport with residents, families, and staff
- Excellent organization and documentation abilities
Compensation
$23.00 – $28.00 per hour, based on experience
