About the Role
Houston Memory Clinic is seeking an LCSW/ LMSW to provide cognitive rehabilitation and psychosocial support to adults with dementia and their caregivers. You will help patients maintain function, safety, and quality of life through structured cognitive interventions, compensatory strategies, and caregiver education as part of a collaborative team.

Key Responsibilities

• Complete biopsychosocial assessments and develop person-centered care plans.
• Provide individual and small-group cognitive rehabilitation (memory aids, routines, compensatory strategies).
• Offer supportive counseling and psychoeducation for patients and caregivers.
• Facilitate family/caregiver meetings on safety, daily care, and planning.
• Coordinate community referrals (support groups, respite, legal/financial resources, etc.).
• Collaborate with neurologists and other team members; document in EHR.

Qualifications

• MSW from accredited program.
• Current Texas LCSW, LMSW license.
• 2+ years clinical experience (older adults, dementia, rehab, medical, or behavioral health preferred).
• Strong interest in dementia care and cognitive rehabilitation.
• Team-oriented with excellent communication skills.
• Bilingual (English/Spanish) a plus.
