Required Skills & Experience: -Five (5) years minimum of clinical experience as a Registered Nurse. Experience with Care Coordination, Utilization review and discharge planning Computer literacy.Leadership experience.

Required Education: -Associates Degree (ASN, ADN) from an accreditated Nursing Program. RN Licensed as a Registered Nurse in the State of New York. -PRI and Screen certification within three months of employment. BLS (AHA)

Certifications : -Case Management Certification.

Overview
We are seeking a compassionate and dedicated Care Manager to join our team. The ideal candidate will play a vital role in coordinating and delivering comprehensive care services to individuals with behavioral health needs, addiction challenges, and social service requirements. This position offers an opportunity to make a meaningful impact in the lives of clients by providing support, guidance, and advocacy within a collaborative environment. The Care Manager will work closely with clients, families, and community resources to promote recovery, stability, and overall well-being.

Responsibilities

• Assess clients' needs through comprehensive evaluations, including behavioral health, addiction, child welfare, and social determinants of health.
• Develop individualized care plans tailored to each client’s unique circumstances and goals.
• Coordinate services across multiple agencies such as Medicaid, Medicare, Section 8 housing programs, and workforce development initiatives.
• Provide crisis intervention and crisis management support to clients experiencing urgent or emergent situations.
• Utilize motivational interviewing techniques to encourage positive behavioral change and engagement in treatment plans.
• Offer supervision and mentoring to team members and trainees to ensure quality service delivery.

Requirements

• Proven experience in social work, behavioral health, or related fields; supervising experience is preferred.
• Strong skills in crisis intervention, crisis management, and addiction counseling.
• Knowledge of behavioral health issues, child welfare systems, Section 8 housing programs, Medicare, and workforce development resources.
• Excellent communication skills with the ability to motivate and mentor clients effectively.
• Ability to handle sensitive situations with professionalism and empathy.

Job Type: Contract

Pay: $2,100.00 - $2,400.00 per week
