Position Overview
The Care Coordination Manager oversees the full care coordination process from the initial inquiry through permanent scheduling and ongoing service support. This role serves as a key liaison among clients, families, caregivers, and internal teams, ensuring timely communication, smooth transitions, and consistent, high‑quality care.
Key Responsibilities
- Serve as the primary point of contact for client inquiries and referrals, responding promptly and professionally.
- Lead the intake and discovery process to accurately assess client needs and preferences.
- Guide clients and families through the transition from inquiry to active care.
- Coordinate admissions and collaborate closely with nursing, scheduling, and operations teams.
- Maintain ongoing communication with clients and families, addressing questions, requests, and service changes.
- Partner with scheduling coordinators to align caregivers with client needs for long‑term success.
- Support the onboarding and engagement of start‑of‑care teams.
- Participate in community outreach and relationship‑building with external care partners.
- Deliver exceptional customer service in every interaction.
Qualifications
- HHA, CNA, or LPN certification preferred.
- Minimum of two years of home care or healthcare experience.
- Prior experience with staffing and scheduling in a home care environment strongly preferred.
- Strong organizational, communication, and leadership skills.
- Ability to work independently, manage multiple priorities, and adapt in a fast‑paced environment.
- Comfort learning and utilizing scheduling and client management systems.
- Demonstrated professionalism, compassion, and commitment to quality care.
