Care Manager

Job Summary:

The Care Manager provides care coordination, utilization review, and discharge planning support, drawing on a minimum of five years of clinical nursing experience. The role requires strong computer literacy, collaboration with interdisciplinary teams, and compliance with New York State licensure and certification requirements, including BLS and PRI/Screen. Leadership experience and case management credentials are preferred.

Location: Bronx, NY

Hours/Shifts: M-F - 10:00 AM-6:00 PM (02/11/2026 – 05/16/2026) Likely to extend

MUST HAVE REQUIREMENTS:

• 5 years minimum of clinical experience as a Registered Nurse.
• Experience with Care Coordination, Utilization review and discharge planning computer literacy.
• Associate’s Degree (ASN, ADN) from an accredited Nursing Program required.
• RN Licensed as a Registered Nurse in the State of New York required.
• PRI and Screen certification required.
• BLS required.

PREFERRED REQUIREMENTS:

• Bachelors or Masters in Nursing
• Case Management Certification
