Health Care Coordinator
Summary of Position
The Health Care Coordinator provides comprehensive clinical and administrative nursing services for participants enrolled in the Adult Day Health and Overnight Respite programs. This role delivers direct patient care, conducts health assessments, coordinates and monitors individualized care plans, and collaborates with interdisciplinary team members to ensure high‑quality, participant‑centered care. The Health Care Coordinator also serves as a clinical resource to staff, caregivers, and healthcare providers, and provides administrative oversight and scheduled clinical on‑call coverage
Duties and Responsibilities Clinical Care & Care Coordination
- Conduct pre‑admission health assessments to determine program eligibility, identify health needs, and develop individualized care plans
- Implement, coordinate, and oversee healthcare components of each participant’s service plan
- Delegate nursing tasks to qualified unlicensed personnel and maintain schedules and daily census for the Overnight Respite program
- Monitor participant responses to medical treatments and nursing interventions; revise care plans as needed
- Provide first aid and emergency nursing care as required
- Accurately and timely document nursing assessments, care provided, and participant responses
Health Monitoring & Personal Care
- Ensure health and personal care services are delivered in accordance with each participant’s service plan
- Assist participants with activities of daily living (ADLs), including feeding, ambulation, and toileting, as needed
- Conduct ongoing health monitoring and documentation, including:
- Vital signs
- Weight
- Dental health
- Nutrition
- Hygiene
- Identify and respond promptly to changes in participant health status
- Notify families, caregivers, or responsible parties of significant health changes
Communication & Collaboration
- Collaborate with physicians, interdisciplinary team members, caregivers, and community healthcare providers
- Report changes in medical or mental status to attending physicians and other appropriate parties
- Facilitate communication between healthcare providers
- Evaluate the appropriateness and effectiveness of services and make recommendations for continued services or transition to a different level of care
Education & Training
- Educate staff on emergency procedures and participant‑specific health needs
- Provide health education programs for participants at least monthly
- Offer health counseling tailored to participants and caregivers
- Conduct orientation, training, and supervision for employees and students assigned to nursing services
Administrative & On‑Call Responsibilities
- Provide administrative support and scheduled clinical on‑call services
- Ensure compliance with all applicable regulations, policies, and documentation standards
Education & Qualifications
- Registered Nurse (RN), currently licensed to practice in North Carolina
- Minimum of:
- 2 years of experience in medical‑surgical nursing or home health care
- 1 year of administrative or supervisory experience
- Current First Aid and CPR certification
- Strong knowledge of:
- Physical and emotional aspects of aging
- Age‑related diseases and infirmities
- Medications and rehabilitative measures
- Ability to provide at least three professional references, including one former employer, knowledgeable of healthcare coordination experience
Pay: $40.00 - $45.00 per hour
Experience:
- Medical-surgical: 2 years (Preferred)
- Home health: 2 years (Preferred)
License/Certification:
- RN License (Required)
