We are seeking a compassionate and dedicated Social Worker to join our team and provide invaluable support to the residents of our nursing home. As an integral member of our healthcare team, you will be responsible for delivering comprehensive social work services, including psychosocial assessments, counseling, and discharge planning. Your role will involve advocating for resident rights and ensuring the provision of services aligns with the regulations set forth by NYS, Federal, and JCAHO.Key ResponsibilitiesConduct psychosocial assessments to gain insights into residents' needs and challenges.
Provide counseling services to support residents in navigating emotional and social aspects of their healthcare journey.
Develop and implement effective discharge plans, collaborating with interdisciplinary teams to ensure seamless transitions.
Serve as an advocate for resident rights, promoting a culture of respect and dignity within the nursing home.
Act as the admissions and discharge coordinator, managing the process with efficiency and attention to detail.QualificationsMinimum of a Bachelor’s Degree in Social Work or a related field; Master’s Degree is preferred.
At least one (1) year of experience in healthcare, with a focus on understanding the unique needs of the adult and geriatric population.
Demonstrated ability to assess patient data with consideration for age-specific needs.
Familiarity with and adherence to NYS, Federal, and JCAHO regulations.
