Position Overview

The Medical Care Coordinator plays a crucial role in facilitating communication and coordination between healthcare providers, patients, and other stakeholders. This position manages the administrative and logistical aspects of patient care while organizing, implementing, and monitoring healthcare plans to help optimize patient outcomes.

Key Responsibilities

• Coordinate communication between patients, healthcare providers, and internal teams

• Assist with organizing and implementing individualized healthcare plans

• Monitor patient progress and follow up on care needs and services

• Manage administrative aspects of patient care, including documentation and scheduling

• Ensure timely coordination of appointments, referrals, and services

• Maintain accurate, confidential patient records

• Collaborate with interdisciplinary teams to support continuity of care

• Identify and address barriers to care in a timely manner

Qualifications

• Experience in healthcare coordination, case management, medical administration, or related role preferred

• Strong organizational and multitasking skills

• Excellent communication and interpersonal abilities

• Ability to work independently and collaboratively

• Detail-oriented with strong documentation skills

• Experience working with vulnerable populations a plus

• Knowledge of healthcare systems, care planning, or social services preferred
