Discharge Planner

Experteer Overview In this role you work within a multidisciplinary team to plan appropriate treatment and a continuing care plan for patients. You play a key part in discharge planning, helping patients and families choose and use suitable services. You respond to referrals to ensure timely access to care. The position supports smooth transitions of care and coordination across settings, contributing to improved patient outcomes.
