Mon-FriSummaryThe primary responsibility of the inpatient Discharge Planner is to provide coordination of care during treatment, case management and discharge planning services to the patient and family in collaboration with the clinical team in a voluntary, inpatient, commercial insurance and self-pay based setting. Discharge planning includes coordinating the communication and referrals to post discharge providers including home health, residential facilities, transportation, outpatient care or services, and any other type of post discharge provider of services required. The Discharge Planner also accepts alternate assignments, as required. This job requires a critical thinker able to work independently while interacting with patients and their families and the clinical team. This job requires superior interpersonal and organizational skills in a dynamic, fast paced environment.Key Functions
  • Attend inpatient unit and team meetings
  • Meet with patients and communicate with families to assess needs and implement the discharge plan with direction from the treatment team
  • Facilitate communication and information exchange between outpatient providers/referents, aftercare programs/providers and the inpatient team
  • Connect patient and family with other Menninger departments, as needed
  • Provide referrals and information to patients and families and execute the aftercare plan to ensure a smooth discharge
  • Collaborate with other departments such as Finance, Marketing, Quality, and Strategy
EDUCATION: Bachelor’s degree required. Bachelor’s in social work, psychology or a related health field preferred (psychology, social work, sociology, anthropology, human services, etc.)BSW or LMSW preferred.Experience: Behavioral health, inpatient psychiatric or medical case management experience preferred. Experience working with commercial insurance plans preferred.
